Parental Consent & Liability Waiver

I, the undersigned parent or legal guardian of the above-named child(ren), give my permission for my child(ren) to participate in children’s ministry activities and events at Victory Community Church. I understand that the church and its volunteers will take all reasonable safety precautions, and I will not hold Victory Community Church, its staff, volunteers, or affiliates liable for any accident, injury, or illness that may occur during participation in church-sponsored activities.

In the event of an emergency, I authorize the church staff or volunteers to obtain necessary medical treatment for my child(ren), understanding that all reasonable attempts will be made to contact me first. I accept full financial responsibility for any medical expenses incurred.

Photo & Media Release
I give permission for Victory Community Church to photograph and/or record my child(ren) during children’s ministry activities. I understand that these images may be used in church publications, promotional materials, social media, or the church website. I understand that my child(ren)’s name(s) will not be used in any published materials.

Privacy Policy
Victory Community Church values your privacy. Any personal information collected on this form will be used solely for church records, emergency purposes, and communication regarding children’s ministry activities. We do not share or sell personal information to third parties. Photos or videos shared publicly will not include identifying information about your child(ren).
